Digital Diagnosis and Prescription Records for Nigerian Hospitals

Every visit can carry a real diagnosis, prescription, blood group, and genotype — recorded once by the treating clinician and available to authorised staff for every future visit.

A patient's medical history shouldn't live in a folder that only exists at one branch, in one filing cabinet, findable only if the right staff member is on duty that day. On mySmartAccessCard, a patient's diagnosis and prescription history travels with their digital identity.

What gets recorded, and by whom

When a doctor, nurse, or hospital admin attends to a patient, they can attach a diagnosis, a prescription, and — where relevant — the patient's blood group and genotype directly to that visit record. This isn't open to every staff member: clinical edit rights are restricted to doctors, nurses, and admins, so a receptionist or cleaner account can view attendance but cannot add or alter medical detail.

Diagnosis per visit

Recorded against the specific visit it relates to, not as one giant undated note.

Prescription tracking

Medication, dosage, frequency, and duration are stored as structured data — not a free-text scribble that's hard to read later.

Active prescriptions, not just history

Prescriptions are flagged active or inactive, so staff can see at a glance what a patient is currently on, not just what they were once given.

Blood group & genotype on file

Captured once, available instantly on every future visit — critical detail that shouldn't depend on the patient remembering or carrying a card.

Why this matters for continuity of care

Nigerian patients are mobile — they see a doctor at one clinic today and turn up at another branch, or even a referral hospital, next month. When diagnosis and prescription data lives on the platform rather than in a single physical file, any authorised clinician treating that patient starts from an accurate picture instead of a blank page.

This is one part of the same system behind mSAC's hospital attendance and patient identity platform — every visit already gets logged automatically at check-in; this is what turns that log into an actual clinical record.

Bring real medical records to your hospital. Register on mySmartAccessCard — diagnosis and prescription tracking is included, not a paid add-on.
